Primary Care Physician

Job Summary
As a Primary Care Physician, you will report to the Medical Director and be responsible for providing high-quality, patient-centered healthcare to individuals of all ages. You will diagnose and treat a wide range of health issues, manage chronic diseases, perform preventive care, and provide health education and counseling. Your goal is to promote overall health and well-being, focusing on long-term patient relationships.
Key Responsibilities
·       Patient Care & Diagnosis:

·       Provide comprehensive medical care including physical exams, diagnoses, and treatment plans.

·       Manage acute and chronic medical conditions such as hypertension, diabetes, asthma, and others.

·       Order and interpret diagnostic tests such as blood work, X-rays, and imaging.

·       Prescribe medications and treatments in accordance with best practices and patient needs.

Preventive Care

·       Offer preventive care services such as immunizations, screenings (e.g., cancer, cholesterol, blood pressure), and wellness check-ups.

·       Educate patients on disease prevention, nutrition, lifestyle changes, and managing health conditions.

Patient Management

·       Maintain accurate and up-to-date medical records.

·       Coordinate with specialists for referrals when necessary.

·       Collaborate with nurses, medical assistants, and other healthcare providers to ensure continuity of care.

·       Follow-up with patients on test results, treatments, and overall care plans.

Chronic Disease Management

·       Manage long-term treatment plans for patients with chronic illnesses, ensuring regular follow-ups and monitoring.

·       Work closely with patients to adjust medications and treatment strategies as needed.

Health Education

·       Provide patients and families with information on health maintenance, disease management, and healthy living.

·       Develop personalized health improvement plans for patients.

Administrative Duties

·       Complete documentation, billing, and coding as required by healthcare regulations and standards.

·       Participate in quality improvement initiatives within the practice.

·       Attend staff meetings, training, and continuing education as required.

Education, Licensure, and Experience
Education:

·      Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

Licensure/Certification:

·      Valid medical license to practice in the state

·      Board certification or eligibility in Family Medicine, Internal Medicine, or related field.

·      Current DEA registration for prescribing medications.

Experience:

·      Experience in a primary care or family medicine setting is preferred.

·      New graduates with strong clinical rotations are encouraged to apply.
