Physician Assistant / Nurse Practitioner

Reports to:
Supervising Physician and/or Medical Director

Essential Duties:
Patient Care Conduct comprehensive physical examinations and patient histories Diagnose and treat acute and chronic illnesses Develop and implement treatment plans, including prescribing medications as appropriate Perform and interpret diagnostic tests such as lab work and x-rays Provide preventive care, including immunizations, screenings, and health counseling Manage chronic conditions like diabetes, hypertension, and asthma Collaboration Work closely with supervising physicians and other healthcare professionals to ensure comprehensive patient care. Coordinate with specialists and other healthcare providers to ensure continuity of care. Participate in team meetings, case discussions, and quality improvement initiatives. Patient Education Educate patients and their families about treatment plans, preventive care, and healthy lifestyle choices. Provide counseling on diet, exercise, and other lifestyle changes to promote wellness and manage chronic conditions. Administrative Duties Document patient encounters, histories, treatment plans, and progress notes in electronic medical records (EMR) accurately and timely. Assist in the development and implementation of patient care policies and procedures. Participate in continuing education to maintain licensure and stay current with medical practices. Skills & Abilities Strong clinical skills in family medicine. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong problem-solving and critical thinking abilities. Compassionate and patient-focused approach to care.

Education and/or Work Experience:
Master’s degree from an accredited Physician Assistant/Nurse Practitioner program. Current state licensure as a Physician Assistant/Nurse Practitioner. Must have a current DEA registration BLS/ACLS certification required. Experience: Previous experience in family medicine or primary care preferred. Experience with electronic medical records (EMR) systems. Must be able to work on site during assigned clinical hours Physical Requirements: Sit or stand for extended periods of time. Must be able to lift and carry, push or pull up to 50 lbs Must be able to talk, listen and speak clearly in person and on the telephone.

Education and/or Work Experience:
